Generic RCA cover is in good condition, tired looking but no significant tears and no seam splits. No personal writing on the cover or the record labels.

New inner plastic sleeve

Vinyl is in very good condition, visually inspected under a bright white halogen light source and I can see just a couple of very light scratches on either side. Just looking at the vinyl is not a great way to judge the condition so I then played this single ENTIRELY, both sides completely, and there are no issues with surface noise nor clicks n pops - Oh they were the days!
